Filling out the entries for the poultry show

There are 3 of us in our family that are going to show birds in Red Deer in December. 2 are juniors and then myself. Do I submit 3 different forms for us or do I put the kids under my form just with junior in front of their entries?

Each entrant should have a sepperate form.
